Step 1

Connect with us through our website or by calling (943) 284-8131. Our office team is available live Monday through Friday from 8:00 AM to 5:00 PM EST and Saturdays from 9:00 AM to 4:00 PM EST to assist you with any questions or service inquiries. If you reach out during business hours or if a team member is assisting another client, our goal is to return your call within 30 minutes of receiving it. If you contact us online, we will follow up to schedule a call within 24 hours. We prioritize responsiveness because we understand that when it comes to care, timely communication matters.

Phone Consult to Access General Care Needs

Our intake team will work with you to fully understand your unique situation. We will verify your personal information and assess what type of help you or your loved one needs, and provide you with formulated service options that best meet your physical and financial needs.

Schedule Complimentary In-home Assessment

Our team will discuss next steps and schedule a complimentary in-home assessment or in-home conference.

Step 4

As part of our complimentary in-home consultation, a trained nurse or member of our office team will visit the client’s residence to meet face-to-face with you and/or your loved one. During this visit, we take time to understand the client’s unique needs, assess the level of support required, and collaboratively develop a personalized Care Plan tailored to those needs. This process also helps us identify important preferences and compatibility factors when selecting a caregiver. Pairing the right caregiver with each client is an essential component of delivering thoughtful, individualized care.

Once the Care Plan has been finalized and approved, we promptly pair the client with a qualified, carefully selected caregiver and arrange the start date for services.

Senior Clients following up with the Formula admin team

Within 30 days of the client’s first day of care, the client and/or family will receive a customer service call to ensure you are satisfied with the services being received.

FOLLOW-UP CALLS

Directory and nurse will have supervisory visits with client

The nurse or office staff will conduct supervisory visits throughout the duration of care with us. These will be both announced and unannounced visits while the caregiver is present. Plus intermittent visits with the client and the client’s family to discuss progress of care and to identify any care plan changes.

SUPERVISORY VISITS
